I also have the same issue and I've tried scoring one from a cross and a flair pop up. Nothing works. This is frustrating because I've also had issues with other challenges not completeing.

You need to make sure that the RM or LM are not RW or LW also you cannot use CF in the CAM spot. They must all be midfielders. Hopefully this will help.

I have used a squad of players all labeled with LIB and it is still not working. I think some of the special cards are not registering properly for objectives or SBCs.
